Correction of bilateral cross occlusion using an extension device based on the first permanent molars and four mini-implants

A clinical case of correcting cross occlusion in a 15-year-old teenager using a maxillary expander based on permanent first molars and four mini-implants is presented. A thorough diagnosis and this treatment planning was carried out, during which a stable orthopedic effect of palatal suture expansion after growth completion was obtained, i.e. at the stage of CS5 maturation of the cervical vertebrae using the CVM (Cervical Vertebral Maturation) method and at the stage of D maturation of the palatine suture. This period of the maxillary system development is characterized by impaired skeletal growth. In rapid distraction (RPE) with a removable or non-removable device fixed only on the teeth, an expansive force is highly likely not to lead to the desired skeletal increase in the transverse dimensions of the upper jaw due to partial fusion of the posterior part of the suture and will be ineffective for its opening. If transversal pressure is attempted at this stage, a dentoalveolar effect will occur (teeth tilt in the vestibulo-oral direction), which can cause undesirable side effects.
